Welcome Packet

We are excited that you have decided to join the Oxbow community!

In order to participate in Oxbow's programs, each family is required to complete the Welcome Packet. These completed forms can be returned to The Oxbow School's office via mail or email to admissions@xclylngy.net. Please ensure you have reviewed, signed, dated and submitted all the forms on the specific checklist.

It is imperative that both parents and students review the information in the packets. We prefer you email or mail all the completed forms together at one time. Please return these forms in advance of the deadline so that Oxbow can prepare to welcome the students by our program's start date.

The Welcome Packet includes the following forms, please use the appropriate links for Fall 2023 and Spring 2024 below this section:

  • Course Selection Form
  • Dormitory Questionnaire
  • Intro Video Project
  • Health Forms:
    • Health and Permission Statement
    • Medical History and Wellness Information Forms
    • Annual Physical Exam
    • Student Immunization Form (must be completed by your physician within 12 months of the program start date)
    • Copy of Medical ID Card (Student insurance card, front and back)
  • Family Contacts and Off Campus Permissions Form
  • Travel Survey
  • Move-In Day Sign Up Form

Fall 2023

Welcome Packet forms for the Fall 2023 semester are due by July 25th, 2023. In addition to the items below, please complete the Fall 2023 Course Form, the Fall 2023 Dorm Questionnaire, the Intro Video Project, the Fall 2023 Travel Survey, and the Move-In Day Sign-Up Schedule.

Spring 2024

Welcome Packet forms for the Spring 2024 semester are due by December 15th, 2023. In addition to the items below, please complete the Spring 2024 Course Form, the Spring 2024 Dorm Questionnaire, the Intro Video Project, the Spring 2024 Travel Survey, and the Move-In Day Sign-Up Schedule.
